How do I request a replacement health insurance card?USED THE EMPIRE SCHEME
Employees can order a new card to default in the Civic Service website MyNYSHIP at http://www.cs.ny.gov/mynyship Or, if your executive is a BSC customer, please contact the BSC to request repair card(s) by phone, email, mail with fax:
